Your health is important as you rebuild your future. Through our partner Tangelo, you may qualify for 12 weeks of no-cost, nutritious food delivered right to you.


You may be eligible if you have Medi-Cal and live with one or more of these conditions:


  • Type 2 Diabetes with HbA1c > 9

  • Chronic Kidney Disease (Stage 3 or 4)

  • Congestive Heart Failure

It only takes 2 minutes to see if you qualify.
