If you have Medi-Cal and one or more of these conditions:

  • Type 2 Diabetic with HbA1c>9

  • Chronic Kidney Disease State 3 or 4

  • Congestive Heart Failure

You can qualify for 12 weeks of no-cost food. Take this 2-minute survey to find out if you qualify.
